Treating Hypothermia During Liver Transplant Surgery
Hypothermia is a significant challenge during liver transplant surgery, one of the most complex procedures in medicine. Dr. Scott Lindberg, Director of Liver Transplant Anesthesia at Houston Methodist Hospital, shares his experience in overcoming this challenge using the HotDog Patient Warming System.
The Challenge of Warming Liver Transplant Patients
Liver transplant patients are among the most vulnerable to inadvertent hypothermia, which can lead to severe complications, including increased infection risk and prolonged recovery times. Before adopting the HotDog system, Dr. Lindberg and his team relied on forced-air warming solutions like Bair Hugger. However, despite using multiple devices—including underbody, lower body, and upper body warmers—achieving consistent and adequate warming remained a struggle. In some cases, they used as many as three machines on a single patient and still failed to maintain normothermia.
The Transition to HotDog Warming
After switching to the HotDog Patient Warming System, the Houston Methodist team saw consistent and excellent warming results. For the first time, they had to turn off the warming system toward the end of the surgery because patients were getting too warm—something that never happened with forced-air warming, even in their best cases.
Infection Risk Considerations
Maintaining normothermia is critical in reducing wound site infections, but infection risk remains a major concern in liver transplant patients. Dr. Lindberg highlights that infection is the leading cause of mortality in transplant recipients, making an effective and contamination-free warming solution essential.
One of the major drawbacks of forced-air warming is that it required pausing patient warming during the preparation and draping process due to concerns about contaminating the sterile field. In contrast, the HotDog system allowed them to place the warming devices in the desired position from the beginning of the case and keep them activated throughout the entire procedure—including the critical pre-incision phase.
Proven Success in Liver Transplant Surgeries
Dr. Lindberg describes liver transplant cases as the most difficult surgeries for achieving consistent and reliable normothermia. However, since switching to the HotDog system, his team has successfully maintained normothermia from pre-incision through PACU discharge to the ICU, a result they could not achieve with forced-air warming.
